Types of Back Pain: What the Assessment Reveals

Back pain is not a single condition — it is a symptom that can arise from several distinct structures, each with different clinical presentations and optimal treatments. Understanding which structure is involved is the foundation of effective care.

Facet Joint Restriction

Pain that is sharp on movement, eases with rest, and typically localised to one side of the spine. Often worse in the morning or after prolonged sitting. Responds well to spinal manipulation and mobilisation.

Disc Irritation

Central or bilateral pain, often with referred pain into the buttock or leg. Frequently worse with sitting and forward bending. Treatment focuses on decompression, nerve mobilisation and progressive loading.

Muscle Strain

Diffuse, aching pain across the lower back — often after a specific incident (lifting, twisting) or following a period of sustained poor posture. Responds well to soft tissue treatment and targeted rehabilitation exercise.

Sacroiliac Dysfunction

Pain in the dimple of the back or buttock, sometimes radiating into the groin. Common in pregnancy, post-partum and following asymmetric loading. Often misdiagnosed — requires specific provocation tests to identify.

Back Pain Red Flags: Know the Difference

The vast majority of back pain — including severe back pain — is benign and musculoskeletal in origin. However, a small number of presentations require urgent medical attention rather than chiropractic care. We screen for these at every assessment.

Seek emergency care immediately if you experience: loss of bladder or bowel control, numbness in the saddle region (inner thighs/genitals), or progressive weakness in both legs. These may indicate cauda equina syndrome — a surgical emergency.

Seek urgent GP review if: back pain is accompanied by unexplained weight loss, night sweats, fever, or pain that is significantly worse at night and not relieved by any position. These warrant investigation for systemic causes.
